Strategy: Create quality health care for residents in Making Connections neighborhoods by
- Increasing enrollmment in S-CHIP, Medicaid, and other insurance programs for elibible children.
- Reducing barriers to prenatal care and health services through neighborhood-based health navigation and outreach systems.
- Reducing barriers to care and enrollment in supportive programs through the use of a community-wide safety net MIS (medical information system), which would use health data collected on uninsured residents to link hospital emergency rooms with free and subsidized clinics.
- Enhancing resident health and provide capacity to build personal and community health through the use of the 3 Principles Network (formerly Health Realization) approach.
- Reducing the amount of personal medical debt through improved access to institutional financial aid at local hospitals.
- Expanding neighborhood-based pharmaceutical services for uninsured families.
